Quagga Mussel Application 2012 Please download and complete application & take it to any Quagga Mussel Screening location in Lake County.

Lakeport Street Banner Application Form  Applications are being accepted  for Martin & Main at this time, on a first come, first served basis.   A $25 fee is charged for Main Street banners, pay when application is received.  Applications are available for fence banners adjacent to the freeway offramp at Lakeport Blvd, they must not exceed 10 ft. in length.   Please note, you can click on the street banner application above and download it for completion. There is no fee for fence banners.  Please call the chamber at 263-5092 to make sure the location you want is available for the time you would like your banner to be displayed.
